WebCareHealth, a leading remote patient monitoring provider for the management of patients with heart failure, anticoagulation management, and other chronic conditions, announced a partnership with the Illinois Critical Access Hospital Network (ICAHN), a not-for-profit organization focused on strengthening critical access and small, rural hospitals through collaboration. WebCareHealth will serve as a preferred partner to ICAHN and its 57 member hospitals by offering guidance on remote patient monitoring to improve health outcomes, access to care, and revenue streams.
ICAHN named WebCareHealth as one of its strategic partners to support its goals to serve as a recognized resource for critical access hospitals and rural health needs; promote efficient use of information technology services; and offer educational opportunities and resources for its members.

As part of this, WebCareHealth will share best practices and tools that hospitals and health programs can use to more efficiently manage patients with complex conditions through real-time data collection and reporting for reimbursement.
